Unfortunately, things will continue to happen. Protocol or not. The challenger disaster was caused by people not following protcol. Columbia happened because people did not understand the damage a peice of foam could cause. The Apollo 1 fire may also have been prevented. And Apollo 13, if I am not mistaken there was a work order for the faulty part to be removed!
