I've been using Kerbal Construction Time.

When you select to launch, it pops up a crew selection window. 

 

It's a wonderful thing to have.

Sounds like an opportunity for an industrious mod-maker to have the crew portraits show in the VAB/SPH as well as when launched.

 Build awesome spaceplane.  As the last part goes on,  click Launch and Yes simultaneously, shouting "come on, come on" at your PC in anticipation.

Watch it roll slowly down the runway and onto the grass.       No crew.

Revert !

The rocket version involves tumbleweed and little fluffy clouds, though if i skimped on launch clamps you at least get to watch the empty vehicle topple over and destroy the pad.

It happens to me regularly.

Got my "space bus" with 1 pilot and capacity for 9 tourists that involved 4 tourist missions all the way from KSC to orbit Duna only to find out I didn't load "any" tourist aboard.

I had this 2 days ago. It's one of those moments that make you force quit the KSP application put on a movie and forget about what just happened.

Yep, I launched a crewed ship out to Jool and accidentally put Bill in it.

I had to revert back to VAB and redo it.

Stowaways are a bigger problem for me than missing crews, since I launch nearly everything unmanned and when I absolutely positively need a crew for something I usually send them up in a separate SSTO launch (since I'm going to need to bring them back down at some point anyway so I might as well leave a spaceplane docked with my station for the return trip.)

I've been testing an unmanned mission to Moho lately, though (a small mining base using the light, inefficient drilling/ISRU equipment meant to slowly manufacture fuel to allow future manned missions to make their return trips) and someone tries to sneak aboard it every single time.
