I have an 0.625 heat-sheild on my payload bays from sounding rockets,(dev version) and when I ignite my second stage, shortly after the heat-sheild overheats and explodes. mods(that may effect. might be a different mod, but these are the ones in use) vens stock part revamp deadly re-entry continued ferram aerospace sounding rockets EVE (doesn't usualy do this, but shortly after my game crashed.)
