I managed to kill Kerbals in a faulty Space Shuttle that burned up while re-entering the atmosphere of Kerbin. Ok, it probally wasn't faulty and the incident was most likely due to my bad flying skills, But I'm still gonna blame it on Kerbal engineers and the cheap duct tape that they use.
