This isn't what I imagined, and it seems like you still do not understand how Hoffman transfer works.
Basically, because your orbits are too close, the accuracy of interception position becomes veeeery low.
Try to learn how to do interplanetary transfers, or you can consider seeing how mechjeb does it.

1. Rotate your engines. Check if the CoT goes through CoM.

Since your game is not up to date, your mods do not have to be up to date. Find the correct versions, redownload and try again.
Also, try manual installing, CKAN messes with mod installs.

What do you mean by no buttons?
Perhaps some dependencies were missed by ckan? I am not sure about this, but installing mods through ckan may break them.
Did you install module manager?

Well, that IS some serious rocket science. Do you have KER/mechjeb? It helps with the delta V /TWR calculation.
Lets say that your 1st stage has 400 delta v left, and you need to do a burn of 1000 delta v. I use mechjeb myself, so i can see the estimated burn time left. Also, lets assume your second stage has 2400 delta V, and a burn time of 4 minutes. You divide the stage delta V over stage burn time, and that gives you a rough estimation of delta v per second, i.e. 10 delta v per sec. Now, divide the remaining delta v (1000-400) by the delta v per second of the second stage, and that gives you the burning time of the second stage. The same can be done for the first stage.
This method only gives you a rough estimation of burning time required, and has excess time for you to execute manouvers accurately by reducing thrust.

Try to increase the pressure needed for opening chutes. Chutes open as the rocket is rushing through the atmosphere and the pressure is higher. Worked around that one a couple of days ago.
