  1. Entropian's post in Engine plate keeps decoupling prematurely (Kraken) was marked as the answer
You may have set the staging on the engine plate incorrectly.  Check "Advanced Tweakables" in settings, then open the engine plate PAW and it should show an option to disable staging on the plate if that's what you want.  Otherwise...  check yo' stagin'!
  2. Entropian's post in Playing luna multiplayer with Realism overhaul was marked as the answer
I had this problem in RSS as well.  The solution is to use KSP 1.9.1 or above and set the server terrain quality to "Ignore" in the configs.

There's a "death zone" there for non-player-controlled craft.  This is to prevent suborbital "orbits."  Mods such as FMRS let you jump back in time to recover boosters:

Welcome to the forums, @Swurdo.  This is a known bug that's been around for several months.  The bug tracker page is here.
  8. Entropian's post in (ignore) Buggy Landing Gear was marked as the answer
Check your spring and damper settings in the PAW (right click menu).  Try changing the spring and damper strengths; that might help.  If not, the landing gear may not be pointed straight forward, which can be solved using snap mode in the editor.  1.9.1 is causing some odd problems with landing gear, but this kind of issue has been around for years.
