I managed to kill all 6 of the Galileo crew because my shuttle decided that instead of flipping head over heals as happens with lift in front of mass, it decided to spin like a freaking frisbee. Needless to say they were unable to recover due to physics refusing to ever allow a shuttle of mine to reenter.

 

Im moving to SpaceX style reusablity instead of NASA shuttle style.

Just started a new career game in 1.4.3 and I'm trying out the BARIS mod as well.  Still early days where I'm grinding the "test at launchpad" missions for credits and had Bill in a Mk I command pod with just a decoupler and something else to test.  Pressed space bar to stage the decoupler and bang!  Capsule exploded.

A few flights later and I'm ready to try to get Jeb into orbit for the first time.  Had a very simple rocket with a couple of Hammer boosters strapped on.  At launch, one of the hammers failed to ignite and the entire rocket took a hard left towards the VAB.  My VAB is still level one so I don't have the abort action group unlocked yet.  I spammed the staging to try to get everything separated but was too late.

I really like this BARIS mod.

Milgard Kerman: died 5-16-2018. 

Attempted flyby (foolishly approved by mission control, who thought that the crew would find the action amusing) of the lander after jumping off Anomaly Number 1. Subsequently proved why she was a scientist and not a pilot. 

At least she proved that the Mun’s surface is as unforgiving as we thought in the process.

Was found in the astronaut complex by the time the rest of the crew returned.

Eilchelle Kerman: died May 7, 2017.

The effects of heating on her spacecraft were underestimated and a fuel tank beneath her pod detonated, killing her instantly. RIP Eilchelle :( 

May it be a lesson: cheap out on your reentry vehicle. Ditch the fuel tanks. Bring a heatshield.

Nasted Kerman - TOD Y4, D171, 05:02:29

Engine on lander was destroyed upon touchdown on Duna.  A rescue mission was launched, but the second lander touched down almost 20 km away.  Nasted bravely attempted to use her EVA pack to traverse the distance, but ran out of fuel and crashed into the sand.
