Actually it was planned unplanned disintegration, i know too well what could happen if enter EVE atmosphere  
I planned to make some additional measurements within upper atmosphere when probe enter it, but i did not expected that it would burn up so quickly :-( I wanted to juice it for some more scence points :-) 

it never rains but it pours  you might say :-(

Once, I believe before re-entry was actually hot, I was returning from my first fly-by of Duna. I managed to kinda set a return trajectory to Kerbin, but periapsis was too high, and I had no fuel. So I had to use RCS, but I only had four linear ports mounted on sides, not even close to the CoM. So imagine how it looked like.

The proper question would be: Did you ever was so desperate that you used LF/O for mid course correction burns xd?

It can be useful if you need extreme precision, like in a rendezvous when delta V is tight, or when you're still far out from an encounter.
