So I was just looking at the Changes for The Mun on the KSP Wiki and it says "Removed the KSC on the Mun (don't ask)" for v0.14.3 (https://wiki.kerbalspaceprogram.com/wiki/Mun#Changes). This means it must have been added in v0.14.2 which is also the version The Mun's terrain was overhauled and changed from looking like this:

By the looks of it it was most likely a an extra KSC (glitch) accidentally added on the mun in the game in 0.14.2, then quickly removed in 0.14.3.

I had heard of it, but I don't think I'd ever seen pictures of it.

 

Wow, I haven't seen that VAB in a looooong time.  I'd forgotten how ugly it was.

Betcha they added it intentionally to test out launching craft from the Mun to make sure it 'worked', then forgot to remove it.

Yeah, that's the old VAB from pre-0.21. I found these images online when looking for pictures of it. The launch pad appears to be glitchy, like the second KSC on Kerbin

No, that's pretty much how the old KSC looked.   KSC 2, as we have it now, is very similar to the original.   The pic you show is pretty spot on, aside from the grass clipping through the Mun.    You couldn't build big (wide) rockets back then, cause they'd hit the tower upon loading or launch.  You also couldn't reposition the craft in the x/y plane in the VAB to scoot it over away from the tower either.
