A KIS/KAS glitch from accidentally trying to bolt one thing inside another resulted in a mild forceful ejection, in turn causing all 7 kerbals being ejected from their ladders and expelled into space away from the shuttle.  The poor pilot could do nothing as he waiting for them all to puff back in - one at a time for some reason - hoping they all had enough MP.

Thus making the second manned flight of the RKS Shuttle a mostly-not-failure.
