Has anyone had an accidental orbital collision between two vessels that were not supposed to be on rendezvous trajectories?

Oh dear.  My closest is now less than 100m, although I can't be sure how close it was, the other craft went by so fast.  I had a base in the Mun canyon, and I refueled an LF tanker and a spaceplane there.  Pictured here:

I launched them both to a prograde (or so I thought) 7km orbit (as low as I deem safe) within a few minutes of each other.  Only, there's something a little fishy about the spaceplane's NavBall here:

All I saw was a huge white blur zip really close and the distance marker went into the double digits.  I just hate how the red 'N' marker is actually on the southern side of the NavBall.

It's to tell you which way your control point is facing, not which way the navball itself is facing. Relative to the navball, all the markings and colors are actually inverted - for instance, if your craft is in a 90 degree dive, you see all brown on the navball, but that also means the ground side is facing up at the sky.

It's my own fault for not taking care of space junk.  This was at the start of the transfer burn going back to the Mun.  Flown by hand into the parking orbit (no mechjeb).  In this save it was the 2nd trip to the Mun, which means there was just this single piece of debris in orbit.  However the parking orbit altitude was the same as the first trip.  But still, what are the odds :o
