Kerbals Killed Quick.

The aim of this challenge is to uh... Have unfortunate accidents involving... As many Kerbals as you can as quickly as possible in career mode in a plausibly deniable way by the soonest in game date.

 

Rules:

1. Nothing should be clearly designed as a Kerbonaut execution method. That is to say everything should look like it's intended to actually be a useful piece of mission hardware.

2. All kills should be explainable as accidents.

3. Don't make the same "mistake"  twice in the same situation.

4. No terminating Kerbals from the tracking station lol.

5. No killing multiple Kerbals on the same vessel at the same time with the same method.

6. Don't change the heating and such.

7. No cheats.

 

Difficulty levels:

Inept: kill 10 Kerbals. Quicksave and revert is allowed. Plasma blackout, G lock, and pressure limits are all optional.

Negligent: kill 15 Kerbals. Quicksave and revert is allowed. Pressure, G lock, and plasma blackout are not. Kills in the light green flat square around KSC don't count. Having obviously insufficient delta-V from the start is forbidden as a killing method.

Manslaughtering: kill 20 Kerbals. As Negligent but no quicksave or revert. Kills on or in Kerbin's surface or water do not count. Deliberate staging mishaps are forbidden as a killing method.

Murderous: kill 25 Kerbals.  As Manslaughtering but deaths within Kerbin's atmosphere don't count. Nor does simply forgetting to equip and use mission-critical components.

Interesting idea,  so, it can't be an obvious deathtrap, can't use same method twice, and can't have multiple kerbals in one vehicle? 
Sounds like it would take a while, what are your rules around timing?

No rules around timing. Feel free to launch missions as fast as you want, even while other missions are still happening. But it's ingame time and date that counts for the closest thing there is to a score.

You can have multiple Kerbals in one vehicle if you don't mass-kill them all at once by the same method. "Comedy of errors" or "Final Destination" missions where everyone or most die but for different reasons are absolutely allowed.

The main thing that isn't allowed is loading up a jet with 15 Kerbals and veering it into the SPH at half the speed of sound. Or at least that counts as one kill, not 15.

When you say 'not allowed', do you mean that these options have to be set to normal, or are not allowed to be the 'cause' of a death?

Also, what kind of documentation is required? Video, photo, or are we going by the honor system?

 I was thinking honor system but photos would be good.
