I've noticed when you attach a girder radially it doesn't align the attachment point correctly. It's hard to explain so I've included a picture. I've noticed that very simple rockets (beginning a new career) without asas will tend to spin when you have boosters attached in this way.

Multiple girders can be attached end-to-end without a problem (i.e. no rotation) so the node icon's tilt doesn't appear to affect the node itself.

If you're very early into career mode (as in, 1st or 2nd flight) boosters will cause a rocket to rotate even with SAS because without struts they won't stay in place (and will usually bend inwards slightly).

It looks to me like it's centered properly, just rotated a bit judging by tater's picture. I haven't noticed any alignment issues when attaching girder segments to each other so I suspect it is not a big deal. Though I bet I'll notice it all the time now that it's been pointed out, like kerbals having slightly different sized eyes.

I have noticed this to, easier to see with the small girder, however for me its only an issue then I don't use angle snap.

Rotation of the attachment node is irrelevant for radial attachments. The node is not used in that case. And the girder appears to be aligned perfectly in that picture (horizontal/vertical).

That of course does not mean it is fine, I'll take more detailed look at it when I get home.

I made an experiment and I don't see any problems here - the only misaligned thing is the green sphere.

The test construction uses all kinds of attachment: axial, radial to the end of the girder, radial to the side of the girder, and both radial either with angle snap on and off. They're all perfectly aligned on the other end.
