Unfortunately, things will continue to happen. Protocol or not. The challenger disaster was caused by people not following protcol. Columbia happened because people did not understand the damage a peice of foam could cause. The Apollo 1 fire may also have been prevented. And Apollo 13, if I am not mistaken there was a work order for the faulty part to be removed!

Deaths are not fun for family and friends, but people can still learn from failure, wich keeps the death rate lower and lower, untill its as safe as sitting on a chair.
