Docking ports just will not connect

A common mistake is to have one of the docking rings placed backwards. Can you perhaps share pictures of your docking attempts?

In addition to what @Vanamonde said, if your RCS and SAS is active, they sometimes prevent your vessels from self-aligning.  

It looks like you nailed a perfect docking.  Try turning off SAS and RCS right at the moment the vessels start to attract each other.  

Turn of RCS:  'R' key.

Turn off SAS: 'T' key.  

If you have an excessive amount of reaction wheels, they will hold the vessels in their orientation, instead of allowing the two vessels to self-align during docking.  I know this, because I also put a lot of reaction wheels on most of my vessels..

your docking ports are placed correctly, but you have two big ships that are not perfectly aligned. in this case the magnetic docking ports will align the ships, but it will take time. and it won't happen if rcs and reaction wheels are holding the shops in place, so, as @18Watt aleady said, you have to turn them off. preferably on both ships.
