Frequency: Happens every once in a while; in about 25 hours of play, I encountered it in two separate instances, but it was fairly jarring.
In this instance, I was attempting to timewarp through an ion engine burn maneuver (a very long one, ~2.5 hours). However, if I hit the timewarp button, the engine would immediately cease to have an effect on my orbit, and my apoapsis would stop changing. After changing back to 1x on the timewarp, the apoapsis would begin changing again (decreasing, as I was doing a retrograde burn). I saved and exited to the main menu, then quit the game, then went back into the game, and the issue was still happening. Then, the throttle was stuck at 100%, as shown in the screen recording. I had to right click on the engine and deactivate it manually to be able to shut off the engine.
The first time I encountered the stuck throttle bug, I was launching an entirely different craft with different engines from the KSC, and it happened probably at 60 km above the ground. I reverted to the KSC and launched again, and it didn't happen again.
