I had this same issue building a vessel in Orbit, Transferring fuel into one tank, accelerating slightly and then redistributing fuel fixed it. It seemed to happen when a new part was docked. I also noticed when it was orbiting without the Orbital line that going to tracking station would cause the vessel to lose all forward momentum and fall out of orbit.
