Depends on if I am flying or if Mechjeb is flying. If I'm flying, I start the turn (5-10 degrees) as soon as I clear the tower to generate separation from the KSC facilities as they can 'sploda. I started this before exploding buildings to get into the habit. Whenever I use Mechjeb, to save myself time, I set the ascent profile to start the turn as early as I can, which is usually 3 or so km. So far, I have not 'sploda a building yet.

As a FAR user (and the fact that most of my rockets are a little bit overpowered) I begin my gravity turn at 500m.

MechJeb and NEAR user here, I start at 345m (the new lower end of the auto-turn slider in MechJeb dev build 405) with a 55% slope and -4 to -16 degree final FPA.

If your rocket is 10km up traveling 10 m/s, your gravity turn is the least of your worries. Likewise with 3km/s (though admittedly they're different problems).

I've done the former in stock aero (lifting heavy payloads w/ jet-only or RAPIER/B9 4.x SABRE first stages) and the latter in NEAR and stock, though not (always) deliberately.
