Graphics Glitch/Game Crash followed by Lost Vessel & Flag

KSP Version: v1.0.2.830 Windows 64-bit

What Happens: Landed on Mun, unlocked EVA Surface Sampling from the R&D building, went back to landed vessel and EVA'd Jeb. Everything was fine until he landed, then the screen went blank and UI writing went blank (altimeter background was still there). Went back to the Space Centre where the same thing happened but now only the skybox was visible.

Restarted the game and the landed spacecraft, Jeb and the flag I planted had disappeared.

Mods / Add-Ons: All Stock

Steps to Replicate (assumed, haven't tried replicating):

1) Land on Mun (or probably any body).

2) Go back to Space Centre, unlock EVA Surface Sampling.

3) Go back to landed vessel.

4) Exit the vessel.

Result: When Kerbal lands the UI/game becomes unusable, after restart the craft, kerbal and flag will have disappeared.

No crash log as the game didn't 'crash' fully.
