In a pure, traditional patrilineal name inheritance system, in a closed system, names go extinct over time (families that only have female children won't pass on the name - same would be true in a matrilineal inheritance system with an all-sons family too of course), so given enough time without any external input, it's entirely possible that eventually there'd only be one surname.

Of course, sensible people in that situation will go, "this is a useless system", drop the superfluous last name, and switch to something else (or restart it with a patronymic/matronymic system; Billy Bobbington, Son of Jebediah or Jebediahson). So..what is this 'Kerman' business? Is it some degenerate last name? Maybe it's a useless title, like esquire? Or is it some weird thing where the KSC is actually run by a crime family?

Anyhow, yeah - having the same last name doesn't automatically mean they're part of the same immediate family.
