In real life they have contol over the angles of the jet nozzles. Ours are alwats parallel to the craft. Now if they gave us 45 degree 4 way rcs jets, problem solved. But, they haven't done that yet.

It's also a matter of design philosophy if you wanted to be efficient you probably shouldn't have ladders running the length of your craft for any reason. The only cases where you would have to deal are landers and stations. basic ships can stuff all that business into the service module area and not have to care. On a station you'd rarely maneuver and carry extra fuel for everyone so you have monoprop to spare. Landers are trickier, but the with lip you get from the mk1 landercan and retractable ladders you can put a ladder over the thruster and retract it when it's time to dock with no real issue.

But then you can't use a ladder, because your RCS jets will be in the way. The RCS jets are the least forgiving placement object on the craft. The Mk 1-2 is the only one in stock that actually makes sense.

I always make the 5-way RCS block on those parts. Works perfectly fine. Though it would be nice if we got a 45 degree RCS thruster.

Or an RCS block with five nozzles.

5 nozzles doesn't help on an X (45 degree) setup, you still waste mono unless you have 45 degree RCS blocks.

Unless you don't do that and build in '+' pattern instead. Then you need only two of them each side instead of four and they don't get in the way when climbing the ladders.

I always point my RCS away from solar panels, etc, BTW, even if I don't actually need to.

I do the exact same thing whenever I can.
