Yep. Same here. And second launch, I opened the chute when the engines cut out. Tore it right off.

As for my first airplane...I went through an hour of redesigns before I could get something to pull up and fly straight, but is still started pitching uncontrollably shortly thereafter. Eventually, I gave up and found a tutorial.

Totally this. Although I knew what I did wrong.

What I didn't know was that I could revert, so I built the same vessel again with better staging instead of just reverting to launch :D 

 

(This was in .23 at the end of 2013, by the way.)

After reverting several times due to staging err incidents, my rocket finally took off. When the engines burned dry after a short tumbling flight (didnt knew of SAS), i reckognized, that parachutes would have been a good idea.
