Undocking glitches makes space station with docking ports unplayable

I am trying to build a space station with docking ports but undocking is making the ship unplayable.

Here are the observations:

1. Most of the time, undocking separates the ship at first, but after a few meters of separation it docks again, displaying the flight report (see screenshot)
2. In some cases, it shoots the smaller ship far from the main ship. Meaning after undocking, the next second shows the ships being kilometers apart.
3. In rare cases, it's similiar as the previous point but the camera far and stuck from the ship.
4. This maybe unrelated to docking ports but after undocking with the ship loosing comnet connection, it spins out of control.
