never happened to me before and i think it isn't supposed to happen either. If you don't have the steamversion try a new clean install, might be a bit "overkill" but it should fix your problem 100% ^^ Did this happened before or is this your first time launching?

Not my first time. It seems to become popular with the new patch. I have the steam version as it was on sale last week. Am I able to do a clean install?

- Yes, just right-click on KSP in your Steam Library and select 'delete local content.'

- Then, go to your Steam download directory (default is C:\Program Files (x86)\Steam\SteamApps\common\Kerbal Space Program - on a windows system IIRC), and delete any remaining contents (this will delete any saves you have).

- Then, simply re-install KSP

I've done the above procedure and the problem still persist. This time it doesn't happen right away. It happens near leaving the atmosphere.

Either it is a glitch or Kerbals get killed due to G-Force:

Here is their facial expression as I ascend into space.
