Kerbals flying out of craft uncontrollably

I'm having a problem with a ship. Whenever I send my Kerbals on EVA, they fly out into space uncontrollably. The ship isn't rotating fast or anything. And it only happens with this one ship. Any ideas?

Do you have anything like a radial chute, light, or landing gear anywhere near the hatch? I find that if I put a light or really anything that isn't a ladder near their foot holds they tend to get shot away from the ship when going eva in space.

Yep, that's been my experience - some object clipping the hatch.
