I just landed at the moon and I EVA with Jeb and took a surface sample then I put Jeb back in the pod and let Bob out he was falling to the ground because the ladder was too difficult for Bob to use and boom this happend :

Really? how could this happen??

EDIT :

Okay I tried it again and the same thing happend but this time with Jeb, also when I quick load the screen stays black and I have to restart the game.

It appears to be some kind of bug.

FAR and Deadly Reentry have this.

Otherwise, yes, I believe each and everyone of our astronauts should give an collective "OH CRAP", and proceed to buy life insurance for their families.

Then it's that.

Deadly Reentry will kill your kerbals if they are exposed to extremely high G forces/reenter at wrong angles. Also, the reason everything is 66666 is the hell kraken,in which I believe a collision mesh and your Kerbal literally falls through the skybox and out of the universe, exposing him/her to superhigh G forces...

Restarting the game usually fixes this sort of glitch. While it can be brought out by some mods, this issue does exist in the stock game.

Getting off a ladder is one of the most dangerous things a Kerbal can do. If you're unlucky enough to step on the wrong spot of ground, you can fall through the world like this and unleash the Hell Kraken. And even if you find solid ground, you can trip, roll under your ship, and then flip it over when you stand back up. For these reasons, I've taken to putting my crew compartments on the bottom of the rocket to avoid having to use ladders .

I once had Jeb climbing down the ladder and he appeared to headbutt the lander three times in a row.... Lander flew off at ridonculous speed leaving Jeb on the floor wondering what he did.

Yeah, the physics in this game can get a little wierd at times.

The original 3 will re-spawn after a time IIRC. Not sure if deadly reentry changes that or not but I do not think it does.
