SAS sleeping on the job?

So I was building some planes when I realise that sometimes SAS just doesn't kick in to keep the plane pointing straight after I performed some simple maneuvers.

As seen in the picture below, the SAS mark is in the orange-arrows, which, to my understanding, indicates that the craft is taking user input(Feel free to correct me), but the issue is, my hands wasn't on my keyboard(I don't have a joystick either). Is there something I can do to fix this?

Ah, the good old spin-outs. The SAS can't control the spin so it is overriden, hence the yellow symbol instead of SAS.

Could I see some more pics, with Center Of Lift, Center Of Mass, and Center of Thrust activated?

Here's a tip: MOAR Control Surfaces!! Also, lose those two angled Advanced Canards.

Oh, that happens when your spacecraft is experiencing large G-forces from maneuvers. If the SAS is not powerful enough to correct your spin, it will attempt to find an area which is between the G-forces and your last position. I suggest executing slower maneuvers or learning to fly without SAS. (It's challenging, but I've done it.)

The orange arrows symbol means that you are in "damping mode", it doesn't necessarily mean that you are currently giving manual input. On that picture you can see that your pitch and yaw control authorities are maxed out, meaning the plane probably can't maintain control and can't switch back to "locking mode".
