How to make a stock hinge

Not possible to do it cleanly in stock KSP, no such thing as hinges. There are very hacky ways of doing it, some of them interesting, but they all stand a good chance of attacking a Kraken to visit.

Use mods if you want a clean solution to it.

There's only one way to do it that I know of, and it's really extremely hacky. Basically girders as hinge pins, attached to door, but not body, cubic struts forming a loop around them (attached to main body only, not door). Docking ports to hold the "door" shut. Probe core to control the door-ship once you decouple it from the main ship. It's clever and interesting, but far from a clean way of doing anything, and stands a very good chance of failing from a physics glitch, or inviting a Kraken to come and munch on your ship. I absolutely do not recommend using that hack, it should be regarded as an interesting curiosity, not a good ship design which can be relied on, in my opinion. You're far more likely to run into problems using that hack, than by just using a good mod pack which does it cleanly. It's also the sort of thing that's very likely to break across KSP versions.

There was a good example of a shuttle door hinge in the KerbalCon stream, though I forgot who was broadcasting.

A door made of structural panels, and a few cubic octagonal struts to attach an Oscar-B fuel tank. The Oscar-B tank makes the "hinge pin" which is caged in a "socket" made of cubic octagonal struts which are attached to the main craft.

The attachment was made with docking ports so it could be dettached and reattached (also docking ports on the outside so they would "lock" open".

The door was pushed open with landing gear bays.
