How do you do your intercepts/transfers to other bodies?

I try to get the intercept approximately correct on the escape burn (I drop down from Minmus when going interplanetary and burn at a low Kerbin periapsis) and then make midcourse corrections (usually a plane change).

After I'm in the target SoI, I usually still have some final corrections to make, usually cleaning up to get a nice equatorial orbit (in the appropriate direction, if I plan on visiting satellites) and/or aerobraking maneuvers.

I've successfully used this to get to Duna & Eve. My Moho probe used a slight variation on this method. I haven't even attempted to make it beyond Duna yet.
