I don’t think so- Minmus looks like a captured comet rather than the remnants of something that collided with Kerbin.

It’s quite possibly a supervolcanic caldera, similar to Krakatoa or Yellowstone.

Yes you are probably right. I think as well for the distance it orbits from Kerbin and it's size/mass, the impact required for it to end up there would have probably easily destroyed it as well. 

That's an interesting idea, perhaps not entirely extinct either...
