This is also happening to me, I think it might be related to crafts that have been docked/undocked prior to deploying the parachutes since it never happened to me when I built simpler crafts for easier missions. But as I made more complicated crafts with landers and whatnot, parachutes have never deployed properly since. The only work around I've managed to find is to make a save, return to the main menu, and then reload the save. Reloading the save in game will not work, you have to quit to the main menu or quit the game entirely for this to be fixed via reloading.
