Disappearing Docking Port

So, I've been trying to dock a new space station I set up around the Mun that includes a ore mining & converter outpost so I can see how it works since I'm still trying to understand how effective or ineffective some of this stuff is.

Anyway, I have a relatively large rocket I'm trying to dock into this space station because the primary point of it is to be simply a large fuel tank for other ships to refuel off of, replenished by the ore conversions. I'm able to make it close, but then I run into an issue, which you can see by the pictures.

The first picture shows me getting cm away from the docking port (albeit at the wrong attitude, which I've thus far been unable to fix since I put my RCS thrusters in the wrong spot and thus they don't turn my craft around like I want it to... and also because I'm horrible at docking)

All well and good, right? I can just keep nudging it until I get the attitude correct and then we're golden.

But this keeps happening:

Without me impacting it (not that a 0.1 m/s impact would do anything anyway), the docking port randomly disappears off of the hub like it was destroyed and I'm not sure why. No explosion, nothing, it just disappears and the space on the hub becomes empty.

Does anyone know why this is happening or what I can do to fix it? I've already restarted the game and all that.

I see you have a few mods there, so I've moved this to the modded support forum.

Thanks for posting a picture, that does help. I don't know this as a stock problem off hand, so possibly it's being caused by an add-on. Which add-ons are you using? Also, it might be helpful to check your log file at that time and see if something is coming up in there.

Only mod I have is Kerbal Engineering Redux, if that's causing any issues (which it didn't seem like it was prone to doing), then I'll gladly remove.

How do I check my log files?
