One off the holding locks didn't open and the the stage didn't detach. The orbit's closest point is 65 miles away.

As it was the soyuz 2.1v, guess we are seeing the teething issues of the volga upperstage here (as it's devellopment was started back in 2008)

 

Though, i wonder why the journalists had to mention the manned version of soyuz... current Manned soyuz flights take place on the much older soyuz-FG (which is not even concerned by volga...)

The bolt that didn't fire, and apparently the rest of the payload interface, was built at the old PO Polyot plant, now a Krunichev subsidiary; so it looks like yet another Krunichev QA issue.

Will there be anything left to recover? As this is considered a military satellite, I imagine they would want to recover what they can.

"Crash"? They should've said it's a russian attack on something. More drama!

It's simply burned up yesterday (just five hours after the OP), the last tracked position was 35S 1W.
