I send a probe into the Sun.

Bombmites discover your warhead and eat all your charges, creating a dud.

A mission to verify gravity, build a rocket (the payload is the omnipotent space-time stabilizer) with a beam attached from the bottom the rocket engine node, falls over and fuel tank explodes.

kerbal cannot read greek gets confused and steers into moho.

I tell the kraken where your kerbal fell, he smacks his lips and gulps, from his burb we hear 'i am the omega'.

I'm not sure that's a space mission, but: The Kraken is shape-blind, so he confuses Moho with Bop and eventually dies there.

Hit by an EMP and goes off course striking an fortuitous asteroid passing close to Minmus.

Mission is to test new flame retardant spacesuit for Clabdirt Kerman by tying him to the launchpad during the "flea" liftoff with a battery and octocore on top.

It turned out to be a Ford probe was found on the road dead. 

Sent a tow truck out to pick up the probe.

The tow truck accidentally went too close to Saturn and doesn't have enough delta-V to slow down around Neptune.

I send a mission to catch up to the tow truck (now escaping the solar system) and put it in a bag to bring it back to Earth.
