Depends what you mean by "refuel".  Here are various things you can do in KSP:

  1. You have a ship orbiting Kerbin, and it has a docking port, and it's low on fuel.  So you launch another ship that has plenty of fuel, and also a compatible (same-size) docking port.  Rendezvous and dock.  Then transfer fuel from one ship to the other.
  2. Same deal, but you bring the fuel from a mining setup on Mun, Minmus, or wherever, instead of launching it from Kerbin.

If your concern is about "how do you transfer fuel from one fuel tank to another":

  1. Right-click on one tank (so that its little pop-up window comes up showing fuel content).
  2. Hold down the ALT key and right-click on the other tank.  Now both of them show their little pop-up windows, and those pop-ups have "In" and "Out" buttons displayed.
  3. Click an "In" button on the target tank, or an "Out" button on the source tank, and fuel will start to transfer.

If you're doing the above, but the "In" and "Out" buttons don't show:  that's what happens if you're playing a career game and you haven't unlocked fuel transfer yet.  You get that when you upgrade the science facility past its initial level.
