Findings of the KSA:

The loss of the Domino was a large lapse in ship safety guidelines. The KSA should have flown test flights to assess the landing capabilities of the Domino on the Mun. Furthermore the addition ofpassengers to the system was a massive lapse in morals and safety. The official final position of the Domino on the Mun is unknown. The public cover story is that the Domino suffered a massive failure in LKO.

The findings of the KSA are to remain classified until such time as can safely be revealed without backlash to the agency.
