Wow. And in the same line as the OP was think my first thought was for all the radical vectors and how much of a PITA it would be just to get back under control and back aboard the shuttle. Pure utter wow-ness.

Explosions in space don't make fireballs. The fact that they seem to be trying to be reasonably accurate just makes their mistakes worse.

Depends. There's oxygen within the capsule, and possibly some liquid oxygen within the fuel tanks; they can certainly catch fire.

Just having a SAFER doesn't automatically make it safer. On STS-121, for example, Piers Sellers' SAFER came unlatched during a test of Shuttle repair techniques.

They're redesigning the latches with hard covers to prevent this from happening again. In the meantime, remember that assessing risk is hard -- especially in an inherently hazardous environment like space. The risk of something bad happening on EVA needs to be balanced against the odds of the extra equipment causing something bad to happen on EVA.

All combustion needs is an oxidiser and something to burn mixed at the right ratio, and an ignition source.. If you had say, a propellant leak, say LOX and hydrogen, and something ignited it near the source before the gasses had time to disperse. It'd be quite possible to get a fireball in space... However the event would be extremely short lived as the burning mixture would expand spherically VERY rapidly and quickly lose sufficient fuel density and mixture ratio continue to support the combustion.
