This happens literally every single time if you put a quantum core anywhere between a main stage and an upper stage. The bottom of the quantum core is bugged and the rocket just detaches and passes straight through it. (from the quantum struts mod)

You need space tape. Strut the bottom stage of your rocket to the top and you should be able to launch no problem.

You need struts between the upper and lower stage, I would put one from each of the outer stages to the nuclear outer stages. then one from the central stage to the outer boosters of stage two.

Another problem you might run into is that two orange tanks on top of each other might collapse so you should put 2-4 struts between them.

Nah, I removed all the mods (thanks to 0.20 file structure) to test it again, and it's the same

That was what I wanted to avoid (putting too much struts), but looks like I have no choice now

Then don't put too much. Just a couple in the right place will probably do.
