Decoupler Troubles

I seem to be having trouble with decouplers not separating stages. Its with the medium decoupler and KW rocketry 3 meter one. Any way to fix this?

Nothing happens that's the problem

Screenshot of your ship would probably help. You need to make sure they're attached by the right attach point, i.e. not push it "over" the part to which you're attaching it.

For illustration: the two rockets in the image below use exactly the same parts. The one on the left has decouplers attached correctly. The one on the right has them "inside out", i.e. attached by the wrong end. Notice how they make the rocket shorter and cover part of used parts' surface.
