How do i turn on perma-death ingame?

It's in the persistence.sfs (?) file for your save game, along with all sorts of other options. It's something like LostCrewsRegenerate, you set it to 'false'.

If you really want a challenge you can get TAC life support which requires your crew to consume Food, water, oxygen, and electricity or they die. You can also easily turn on Permadeath.

Alternatively, in your persistent.sfs there is the line

MissingCrewsRespawn = True

change True to False and your crew will never come back!

Well, it is possible to get from the surface of Gilly to Kerbin with just an RCS pack. Probably landed on his head an bounced upon impacting Kerbin.
