Just now i launched my shuttle with a small (ish) mun lander inside, I then launched and noticed it was swaying a lot inside after placing myself suborbital i opened up the cargo bay doors to watch the lander drift out, i then circularized with the shuttle and lander separately and figured out that it must have been a staging error AT LAUNCH and had somehow stayed inside and not blown up the whole time! heres a pic:

Indeed the parts need to be stronger/sturdier. I know joints in ARM got a big fix but the rigidity of the parts needs to be increased more. Doing so will fix multiple bugs in the game as well as make it more realistic. Ever seen the orange tank on the space shuttle flex like a wet overcooked noodle?

If you use mechjeb automatic landing it tend to do a loop then landing on low gravity moons like especially Pol but can also happen on Bop and Minmus, think its related to wanting to reduce speed to 0 before final decent with engines who are overpowered for the moon.

I just started a new career with RSS + RO (that was a pain to get everything working...) and built a simple jet to try to get some easy science flying around. Apparently one of the realism mods in there changes the connection strength of the wings to the fuselage, because I accidentally pulled up a little hard and the wings snapped off. I happened to be on approach to KSC and I had enough airspeed and thrust to keep flying just using the elevators and rudder so I figured I would still try to land it. On touchdown the gear crumpled and the fuselage broke up, launching the cockpit. Just before it hit the ground Jeb bailed and skidded to a stop as the cockpit exploded in the background. More proof that Jeb is the best test pilot ever (and I need some more practice flying planes).

If you have FAR that's not an RO thing, it's a FAR thing. FAR adds aerodynamic failures if you pull up too hard or too fast.
