Usually because there's something close enough to the door that the Kerbal clips it and is flung away, luckily Claw has fixed this in his stock bug fix

It's a known bug.

It seems to happen mostly with the KM1 pod when you attach any of various items to the top of it.

Your post date and join date suggests that you may be a new player.

If you are currently playing without mods, I suggest that you try the capsule on the pad an EVA a few times, going in and out of the capsule.

Then try the capsule with various items attached to the top and see which ones are affected.

If you have only half as much fun as I have with KSP, you will still be entertained for months.
