Whited out icons in main toolbar

I'm having this strange issue where several of the icons I have in my main toolbar are showing up as white squares instead of showing the icon. The icons still work. I can click on them and they open up the right windows. Click again and the windows go away. But it's annoying and I'd like to fix it. Is this a known problem? Is there a fix for it?

Broken thumbnails in the updates, downloads wherever we dl them from, github, Kerbalstuff, course...
I have a broken Kerbal Engineer Redux icon, i could fix it, but it doesn`t bother me, i play very few mods with toolbar icons.

ATM does that, only fix I know is removing it.

Some other common issues:
1) Incorrect install path. Many plugins use a specific GameData relative path for loading the icon. Make sure don't have nested GameData folders or anything like that
2) Reformatted icons. This happened a lot pre 1.0 and native DDS texture support with people changing these icons to DXT formats which could cause the same issue as 1) depending on how the icon was loaded
