try revert to launch and launch it again. it looks estimate few things at first (time takes between launch and achieve orbit, drag, etc..) get to nowhere. and if you revert and launch it again, it has better accuracy by use the datas from first launch

well, at least it works with launch to rendezvous.

Everything works as intended. You just ignored two small matters: timing and location of the required orbit. Mechjeb uses equator of Kerbin and the current location of KSC as the frame of reference. If you don't line up moment of the launch with Ascending\Descending node of required orbit, your actual orbit will be out of alignment - it will have correct inclination, but in wrong place.

But the autopilot was in charge of the launch window.

"launch to match inclination" (or whatever the option is called) DID used to launch at the ascending node you could rely on it to get you within 3-5 degrees of the correct inclination. I've also observed this recently though, I assume it's a bug.

This is NOT a bug but rather PEBCAK. It has happened to me several times before I realized what was happening and I am pretty sure it has just happened to you too.

When launching to match plane two new field open up on the bottom of the ascent guidance. One of them is an offset allowing you to launch a certain amount of degrees before or after the node. If you do not make sure this field is set to 0 degrees you might end up in a completely different orbit. Your inclination will be equal to your target but the planes will not line up.

that field is to be used to tell MJ how much of an offset in LAN your launch trajectory creates. If by the time you circularize your LAN is 10 degrees past what it was at launch, put 10 there. MJ will populate the field for you if you launch and then rvert.
