By drop tanks I mean fuel tanks that can be dropped. By moving the 8 tonnes of fuel in the transfer stage up to the top of the vehicle mounted on a large docking port and discarding the 4 tonne vector engine as I did, the fuel in those tanks will give you enough delta-v to do the transfer burn to the Mun. The empty tanks can then be discarded bringing the lander weight back to about where you had it.

As it is now, the extra mass of the Vector engine and its terrible vacuum Isp (worse than the LV-T45) means that that 8 tonnes of fuel won't be enough to complete a Munar transfer burn. The step down from 2.5m to 1.25m tanks also reduces the stability of the rocket.

Without crew transfer you'll have to add a means to detach the crew module and stick a few parachutes on it (a heat shield might also be useful).

First why don't use probe cores?  this way you can get away with an MK1 pod. 
My main issue with Mun rescue is to land close enough to kerbal, however with the new navbal for eva it should be far easier to do an long eva flying.
My standard Mun rescue craft is MK1 pod with parashute on top. scervice bay with probe, batteries and mod stuff like mechjeb and life suport. heat shield and decopler, yes I could drop the service bay too but this was previously used for science gear.
45 liter tank and 505 engine, then 4 45 liter drop tanks with landing legs, 4 45 liter tanks below the outer tanks for the Mun transfer burn, this is launched with an skipper.

I like manned missions, secondly I'm considering installing KER, Kerbal  Alarm Clock and Precisenodes...It'd just help to be quite honest for a specific landing site etc. I'm not planning on using a Life Support Mod. Secondly...I will be arming the ship with heatshields + parachutes if possible. Not really tried rescue mission before so here goes nothing (hopefully) and never really needed a return stage.
