Losing altitude when moving prograde?!

Are you sure your navball markers are ORBIT prograde and retrograde and not one of the other settings? Is it possible you have "control from here" set to some part that is mounted backwards with respect to the engines?

As far as circularization goes, what you want to do is change the altitude of a node at the opposite one. Both because it is fuel efficient, and because it won't have much effect on the node you are at. Depends. If you have a 30 second burn coming up, start the burn 15 seconds before you arrive. That way, things are balanced.
