Is there a method to determine places you've been? Meaning, if I were to search the Mun for anomalies or whatever, how can I tell where I've been and where I haven't.

Thanks. :-)

Flags are particularly useful for that purpose. You can also re-classify craft (like rovers) as bases and they'll act as ground beacons when you get close enough...I've used a system like that to set up an ILS for KSC's runway...

Kinduva bad pic, but you get the idea. Four of those ground beacons started life as rovers; the rest were probes originally. Re-classify them as bases and you can see them from orbit.

I used flags for all my anomaly markers. It's useful to add something descriptive as the flag site when planting the flag, as this then shows up in the tracking station.

Does the location of anomalies change when you start a new game or are they always in the same areas of each of the planets?

They stay the same. This is true between versions in most cases as well; the NAM was in the same position in 0.20 as it was in 0.21. The developers occasionally do move anomalies...the one that comes to mind is the Mk-I memorial. Also, a few of the anomalies on Kerbin and Mun were buried in the last update (procedural terrain and all; the reason why we couldn't bring savegames over from 0.20 to 0.21).
