My hues are as varied as the rainbow,

but I most commonly bear the shades of pain.

Sometimes I am as sand, sometimes I am as water, sometimes I am as blood.

Bear my vessel upon thyself at thine own risk;

those who are careless shall find their heart is left with a stain.

The "heart" is not literally referring to an actual heart.

It is a tangible thing.

Explanation:

My hues are as varied as the rainbow - pretty self explanatory

but I most commonly bear the shades of pain. - Red (Blood), Black, and Blue (black and blue is a colloquialism for bruises) are the most commonly used colors.

Sometimes I am as sand, sometimes I am as water, sometimes I am as blood. - Ink can be granulated or powdered, aqueous, or very thick (like calligraphy ink)

Bear my vessel upon thyself at thine own risk;

those who are careless shall find their heart is left with a stain. - Stick a pen (vessel for the ink) in your pocket and you might find you have a stain. Unless of course you wear a pocket protector.
