Command Seat Needs Seat Belts

I have an ascent/descent vehicle that only has a command seat - no capsule. I have been using it for several versions of the game.

Recently, Jeb has been falling out of the seat and ends up in orbit with my surface samples. He doesn't fall off during liftoff. He seems to fall off during rendezvous with my space station.

Any idea what is going on or how to overcome this problem?

And what is the best way to rescue Jeb from this situation?

A seatbelt would likely double the mass of the seat.

It shouldn't be too difficult to fly back to the seat using jetpack, unless you fall out while an engine is running and the ship escapes completely.

I guess you could use the little cubic struts to make a cage?

Seatbelts would make the chair to safe. Jeb lives for danger!

how about just fixing it, since if you knock kerbals out of it, they can turn into lifeless statues and forever become broken
