I'd go with the Kraken. My ships usually fail far from home for weird reasons like they suddenly stop orbiting yet refuse to fall, or warp past something I set to warp to, or just randomly teleport to "landed on Sun."

Second is probably Stage Fail, which happens more often but is far less detrimental seeing as it usually happens on launch or while rising through the atmosphere, and I have no qualms about reverting if my SRBs simultaneously ignite and detatch, while tossing out parachutes.

Most if not all of my rocket launch failures have to do with the trajectory with it either being too steep or too shallow like in this pic:

Sometimes, like that, it isn't a total failure and I still get to orbit. Other times, I either waste too much fuel or simply fall back down :(.

The rest of my failures usually happen during reentry, and most of those involve flips leading to overheating and/or aerodynamic failures, though I've found out that putting basic fins on top of the command pod (as you can also see in the pic above) is a cheaty yet working way to prevent flipping. With planes, the only things that regularly go wrong are aerodynamic failures or screwing up a landing. For my rovers, I usually don't get anything bad because I use KerbalFoundries wheels or treads or the stock wheel configs :sticktongue:.

I tinker a lot. My testing sessions can be pretty vigorous. I tweak, launch, remember I forgot to change something, and revert, over and over and over again. Technical failures during testing are to be expected, but the thing that happens more often than not is I revert one too many times and the game has a heart attack. I crash to desktop a lot, but I mostly think of it as my fault. Sometimes I try and revert even before physics has kicked in. I need to learn to take things a little slower :P

Over correcting during the ascent stage and turning into a great ball of fire. I've been taking it very easy lately and my launches have been going much better.

Or getting to where I want to be...and discovering I forgot something...like a docking port..or solar panels. Y'know...minor stuff. :) Like kicking all but whatever Kerbal that is piloting out of the ship when heading out for a rescue mission. Stupid hitchhikers.

I had always problems with the boosters. But then I figured out how to get rid of this:

  1. Just select a single booster.
  2. Empty it.
  3. Have a look at its center of mass (CoM).
  4. Remember where it is.
  5. The next time you use this booster add sepatrons at the CoM.
  6. Start the sepatrons with the separator.
  7. ...
  8. Never having problems with boosters smashing into my rockets again!

I had to put a bit of thought into this, and to be honest, my most fails are literally the Kraken. My 600 part plane has a cargo ramp that as of now, likes to rip itself apart magically. My space station heated up to 17492864242 degrees and exploded (went supernova). Another large craft of mine exploded when I tried to timewarp. Also I recently tried to reach orbit only to find the craft could not move, it ripped in half and then gravity stopped pulling it... Poor space shuttle was dragged off into deep space by the Kraken.

Forget antenna EVERY DANG TIME!

Servcice bays breaking parts off, killing kebals, causing collision issues, shooting a mining lander off of Ike at half the speed of light.

Forgotten ladder. Hasn't been mission breaking yet, but is certainly annoying when you're trying to do science.

Seperatrons have killed launches in the past. Before I realized you should angle them away from your main stage...

Never had the 'forgot to open solar panels' issue. Though I have had the 'not enough solar panels' issue on more than one occasion.

Laziness is my most common type of failure. I just can't be bothered to do things properly, so I make guesses, take risks, and hope that it will work. If it doesn't, I reload and try again, perhaps taking the situation a bit more seriously.
